Sammanfattning

Patients with ischemic heart disease often suffers from diabetes In 2006 a project aimed to improve quality of care of patients with glucometabolic disturbances and ischemic heart disease was started in Malmö. A cardiologist, diabetologist and a prevention nurse confer regularly to discuss patients with poorly regulated diabetes, new-onset diabetes and prediabetes who have been warded at the Department of Cardiology for acute coronary events. Using a retrospective survey design we reviewed all patients discussed at the conferences 2008–2009 (n=224). We observed a significant reduction in HbA1c, measured in conjuction with the conference compared to HbA1c measured one year later, for patients with poorly regulated diabetes (p<0.0001) and new-onset diabetes (p=0.03). Additionally, it is our experience that screening for glucometabolic disturbances among our patients with ischemic heart disease has increased, active adjustments in hyperglycemia treatment and recommendations to primary care have improved and the standard of care for these patients has become more unified.
