Sammanfattning

Transition of adolescents with a chronic disease from the pediatric to the adult clinic is often problematic, and can lead to difficulties for the young patient and his/her family. A real problem for the HIV infected adolescent, is adherence to medication, in order to prevent resistance to antiretroviral treatment. We have therefore set up a transition clinic for young patients with HIV that is held one evening every month. It is run by a multi-disciplinary care team including physicians, nurses and counselors from both infectious diseases and pediatric clinics and sexologists. We have run this clinic for five years and despite difficulties due to immigration, experience of parental death, and adolescence problems, 90% of the young HIV patients who have been on antiretroviral treatment for more than 6 months have viral suppression (i.e. HIV RNA <50 copies/ml). The treatment results for these young patients with HIV are similar to those in the adult Swedish population with HIV.
