The purpose of this study is to examine how admission notes, written by health professionals at a Community Mental Health Center (CMHC) for patients admitted to acute mental health care, illuminate the patient’s perspective. Sixty-six admission notes for twenty-two patients (referral from medical doctor in primary care, handover notes written by a psychiatrist and nurse) were analyzed using qualitative content analysis. Study results show that the majority of the admission notes presented the patient’s situation. The patient’s wishes, needs and goals for the inpatient stay were insufficiently documented. It was also not clear whether information about medication and treatment alternatives had been presented to the patient. Attention among health professionals to document patient involvement and information about treatment planning in admission notes can be one way forward to support user involvement.
