Sammanfattning

Introduction: This study investigated whether unselected patients operated on with total hip arthroplasty (THA) or total knee arthroplasty (TKA) could accomplish a self-developed accelerated track, ANORAK-HH, with a planned length of stay (LOS) of maximum five days. It also investigated their satisfaction with all parts of the track. Materials and methods: 307 patients were operated on with a total of 329 arthroplasties; of these, 243 were primary unilateral THA and TKA and constitute the main material. ANORAK-HH includes a preoperative patient seminar, patient motivation, dedicated staff, unchanged criteria for discharge and a planned LOS of maximum five days. 22 patient characteristics and 11 satisfaction parameters were registered, as well as the number of readmissions within three months. Results: 95% of the patients had a LOS of maximum five days with a mean of 3.9 days. Satisfaction was median 10 (of 10) on all parameters, with a mean score of 9.4 for the entire track. Readmissions were not increased compared to longer LOS. Participation in the patient seminar did not influence LOS or satisfaction. 80 patients had a LOS of maximum three days; operation at the beginning of the week and lack of blood transfusion were associated with this short LOS. Discussion: ANORAK-HH is accomplished by 95% of unselected THA and TKA patients with very high patient satisfaction and no increase in readmissions. This article discusses the factors influencing these excellent results.
