Sodium hypochlorite is the most commonly used and preferred solution for the disinfection of root canals. However, the recommendations differ, not only concerning concentration, but also when it comes to other properties (that are) of importance for the antiseptic and tissue dissolving properties. This review aims at identifying factors that are of importance to balance the antibacterial properties against potential side effects. Concentration is only one factor that contributes to the antibacterial and tissue dissolving properties. Buffered sodium hypochlorite solutions, in combination with elevated temperature, will increase the antibacterial and tissue dissolving properties dramatically. Buffered preheated sodium hypochlorite solutions with concentrations between 0,5?-?1 % seems to best fulfil the criteria of securing antibacterial effect and at the same time minimising the negative side effects.
