Results from randomized controlled trials indicate that screening decreases the incidence and mortality of colorectal cancer. However, a high compliance and accuracy of the screening test are necessary for screening to be cost-effective. Fecal occult blood test (FOBT) and colonoscopy, in particular, have been suggested as screening tests and are used in routine screening programs in several countries. The Swedish National Board of Health and Welfare does not recommend colorectal cancer screening due to lack of experience in screening at the population level. We are lacking knowledge of the effects and side-effects of routine screening in the average-risk population, but we risk never knowing its value if opportunistic screening becomes standard and excludes the possibility of adequate assessment. The Stockholm County Council started a screening program with FOBT this year, and it is appropriate that a multicenter study to evaluate screening colonoscopy is initiated by other county councils.
