The goal of education in medical ethics is usually stated as developing a reflective ethical attitude towards patients and relatives. During the clinical training of medical students teachers are also supposed to integrate education in medical ethics. We have studied medical teachers' ethical attitudes when asking patients to participate in the clinical training of medical students and explored barriers and facilitating strategies by conducting a focus interview with patients, medical students and clinical teachers within dermatology, gynaecology, psychiatry and urology. We found that teachers are genuinely intent on providing good clinical skills in the clinical training, but that sometimes patients and medical students feel uncomfortable if the teacher does not patients and ask their permission. Both the patient and the medical student feel uneasy if the patient's value becomes solely instrumental; even if the patient was an interesting education object from a medical perspective, the students only remember the feeling of uneasiness. We conclude that there is a risk of clinical training as well as education in medical ethics being counter-productive if the teacher acts as a bad role model. Respecting the patients' autonomy when asking them to participate in the clinical training is crucial in all clinical training. We suggest a number of facilitating strategies.
