Platelet inhibitors and anticoagulant drugs are relatively commonly used in the population, causing an increased risk for complications with bleeding when performing oral surgical procedures in these patients. At the Departments of Oral Surgery and Oral Medicine at the Universities in Bergen and Oslo, we have recommended for years that the International Normalised Ratio (INR) value should be lower than 2.0?-?2.5 when performing minor oral surgery procedures in general dental practice. Interruption of platelet inhibitors has not been recommended at the Department in Bergen, in contrast to Oslo that recommended a short interruption (4 days). Disagreements within this topic are also reflected in recent Norwegian literature. In accordance with new national and international guidelines, we recommend that interruption of anticoagulant therapy should only be done in cooperation with the patient’s physician. Minor oral surgery procedures can be performed in patients with INR<3, supported by local haemostasis administration and postoperative information. The surgical procedures can usually be performed without interruption of treatment with platelet inhibitors. However, other potential risk factors for increased bleeding must be taken into consideration as well and may dictate an alternative approach.
