When a patient with a periodontal healthy dentition suddenly experiences loss of periodontal attachment in one site on a rootcanal treated tooth, root fracture is the most common cause. However, foreign body impaction may be a differential diagnosis. Here we report the finding of a composite fragment partly embedded in the soft tissue at the bottom of a deep vertical bone defect. The fragment was detected when performing an explorative flap on the tooth. The most likely source of the composite fragment was the fractured restoration of the neighbour tooth (arrow). Following removal of the fragment, the lesion healed uneventful.
