The term exacerbation is used differently in dental literature, and its prevalence is shown to vary from 0?-?62 %, depending on the criteria used. The material in this study was based on self-assessment forms (n = 2257), filled out for each endodontic treatment by students and teachers in the dental school at the University of Bergen over a six-year period (2000?-?2005). In this study we have defined exacerbation as an acute condition that may arise after instrumentation of an infected root canal (n = 1429). Of the examined forms, 57 were registered with exacerbation. Among these 57 forms, 19 were included according to criteria after a detailed study of the journals. This gave a prevalence of exacerbation of 1.33 % during the six-year period. Routine follow-ups indicated that 66.7 % of the treatments with exacerbations had successful treatment outcomes. Adjusted for cases with non-endodontic failures, the treatment success rate was 81 %. In summary, endodontic exacerbations do not seem to aversely affect treatment outcome. Furthermore, the study indicates that the incidence of exacerbations is within acceptable limits according to the relevant literature, and that our clinical routines seem to be adequate in terms of preventing exacerbations.
