A compartment syndrome is a condition when increased hydrostatic pressure compromises the microcirculation within a limited space. A Crush syndrome is a fulminant compartment syndrome with systemic complications. Normal intramuscular pressure is 2-10 mm Hg. In traumatized muscle the local perfusion pressure ( mean local arterial pressure-intramuscular pressure) must exceed 40 mm Hg. Immediate fasciotomy is the treatment of choice. The abdominal compartment syndrome (ACS) may be a complication to abdominal trauma, extended burns, ruptured AAAs, intestinal ischemia, peritonitis, pancreatitis or massive fluid recussitation. Monitoring of intraabdominal pressure is important in patients at risk. The ACS can be prevented by early conservative treatment, but surgical decompression should be considered if IAP > 20 mm Hg + is combined with organ failure.
