A directly made cast gold inlay may reconstruct, «aesthetically» and functionally, the occlusal surface of inlays, onlays or gold crowns after root channel treatment through the restorations. Construction of directly made inlays is described. Rubber dam is required during the treatment procedures. Deep fillings prior to the cast restorations may be the main reason for endodontic therapy through onlays and crowns in six described cases. Some of the direct inlays have so far functioned for more than 8 years in gold onlays and crowns of the root filled molars. Longevity etc. of different types of fillings in occlusal surfaces of penetrated gold restorations is discussed.
