The concept of elective (interventional) ventilation was introduced in Exeter, UK, and published in a report in Lancet 1990 as a way of obtaining more organ donors. Patients with severe intracranial haemorrhage and hopeless prognosis were provided intensive care from the time of respiratory arrest, given that there were no medical contraindications to organ donation and that consent to donation could be obtained from the patient's family. If organ donation was not an option, the patient was not transferred to the intensive care unit. The number of organ donors increased significantly when elective ventilation was implemented. The concept of elective ventilation was considered illegal in UK and this praxis was terminated in 1994. Since the late 90`s there has been an ongoing debate in Sweden concerning elective ventilation and the various questions that arise when intensive care is provided to acquire more organ donors. This review article summarises the debates in the UK and in Sweden. Ethical as well as legal and medical issues are discussed.
