Our knowledge about the pathophysiological processes following spinal cord injury has increased tremendously during the last decades. The co-existence of various different injury mechanisms has provided the possibility to explore a number of neuroprotective agents in animal as well as clinical settings. The result of this research has, however, been discouraging for patients with spinal cord injury and focus of research has therefore been transformed to the field of neural regeneration. This field includes mapping of regenerating obstacles in the nerve cell and the surrounding environment as well as strategies on how to overcome these obstacles and promote neural regeneration. This review highlights selected experimental and clinical studies either preventing the consequences of secondary injury in the acute stage (neuroprotection) or promoting neural regeneration in the sub-acute and chronic phase.
