Identification of the bony canals and foramina in the mandible is important for avoiding nerve injuries and neurosensory sequelae after placement of oral implants.
The present paper focuses on the anatomic variations of the mental nerve (anterior loop) and the anterior continuation of the alveolar inferior nerve (»incisive nerve«).
Intraoral and panoramic radiographic techniques are unreliable for demonstration of these structures, and even tomography does not secure a reliable presentation.
Neurosensory disturbances following implant insertion in the anterior part of the mandible are very frequent. They are mostly transient, although a few may persist for years. They are most often unpredictable, and it is as if the dental surgeon is acting »blindly«.
The dentist must be familiar with the variations of nerve distribution. Advanced radiographic techniques are recommended, and the radiographs must be carefully and critically interpreted. During surgical procedures the dentist must respect »safety zones« in order to avoid nerve damages. A visual identification of the mandibular foramen during the surgical procedure is very important. Finally the patients must be informed of the possible neurosensory consequences of implant treatment.
