n the present paper side effects related to the mechanical process of orthodontic therapy are discussed. With adequate routines the periodontal ligament, cementum and alveolar bone regenerate in a new position. However, narrow border between adequate and excessive orthodontic pressure, and factors such as duration of treatment and individual responses may result in dentine resorption. The dentine lacunae are usually repaired by deposition of cementum. However, dentin resorption located at the apical pressure side, particularly in long and narrow roots, may lead loss of support for cementum repair. The result is apical shortening, in extreme cases amounting to 1/3 of the total root length. It has been assumed that similar pressure in the marginal area of the tooth may lead to irreversible resorption of alveolar bone and constitute a factor in the development of periodontal disease. However, a series of periodontal surveys of orthodontic patients have disclosed only small average differences between orthodontic patients and reference groups with regard to indicators of periodontal disease. More extensive loss of alveolar bone may occur and is unevenly distributed within the dentition and among patients, and reasons are not fully understood. Considering the inflammatory processes mediated through orthodontic treatment it is impressive that the vast majority of orthodontic treatments are performed without significant sequelae.
