Schizophrenia is a complex and highly debilitating mental disorder. It is potentially treatable, but our current arsenal of medications leaves much to be desired. Antipsychotic drugs that modulate the dopamine system have been the mainstay for the management of schizophrenia. These are classified into first (FGA) and second generation (SGA) antipsychotics on chronological basis. Currently, it is not clear whether SGA represent a major progress in the treatment of schizophrenia compared to FGA. Here we review three large randomized studies (CATIE, CUtLASS and EUFEST) that recently compared SGA and FGA. The major conclusion drawn from these studies is that it is mainly their adverse effects that differentiate the medications and with the exception of clozapine, there does not seem to be significant difference in efficacy between SGA and FGA.
