The article is written based on a review of the medical records of patients with--drawn from the nurse-led Hypertension clinic in Holbæk Hospital Cardiology Outpatient department over a 4-year period from June 2004 to December 31, 2008.
Over the 4-year period, 186 patients were withdrawn from the hypertension clinic. Of these, 130 withdrew directly to follow up by their GP. Ninety-five per cent of these had a set target blood pressure. The patients also had intract-able hypertension and a high-risk profile.
With the establishment of a nurse-led hypertension clinic, a successful role convergence has been achieved, with a well-defined division of roles between nurses and doctors. The individual patients were offered close supervision by an individual nurse, providing the opportunity for guidance, follow up and support in making lifestyle changes.
