The new variant Chlamydia trachomatis (nvCT), with a deletion in the target region for two common NAAT tests, is of epidemic impact in Sweden. This study was performed in Dalarna with initially 67% nvCT, the highest proportion. A survey of patients with nvCT showed the same clinical picture as known with wild Ct. Test-of-cure were 100% negative following routine tetracycline treatment. As infectivity we measured the proportion of infected partners, 58% with nvCT and 62% with Ct. Alcohol habits, age at first intercourse, number of lifetime partners, proportion of anonymous partners and repeated infection were studied for both variants. No difference was found. The high proportion of nvCT could not be explained by higher contagiousity or risk-taking but probably by uncontrolled spreading due to false negative tests. Until now the proportion of nvCT has declined to 24% due to interventions, lag until steady state and maybe biologic disadvantage.
