Sammanfattning

he long-term outcomes in a regional cohort of VLBW children (N=85) born 1987-88 were evaluated at 15 and 20 years of age. Hospital readmissions due to asthma and neurological diseases were more common in VLBW boys than in control boys. Of the VLBW group 6% had moderate to severe CP-syndrome and 6% ADHD. Of VLBW children without moderate to severe CP 23% showed white matter damage of immaturity (WMDI) on cerebral MRI. 1 out of 5 examined VLBW children had IQ<70 on the Wechsler Intelligence Scale for Children (WISC-III). VLBW subjects in comparison with controls showed poorer visual acuity and stereo acuity, visual dysfunction and a higher prevalence of strabism and astigmatism. Visual dysfunction was associated with abnormal MRI findings and low IQ. CONCLUSION: This study confirms previous observations that adolescents with VLBW are at a disadvantage regarding health, mental development and visual outcome compared to subjects with normal birth weight. The findings indicate a cerebral causative component responsible for the cerebral dysfunction seen in the present study. Sequels in VLBW children thus appear to persist in adolescence. However, a majority of the VLBW subjects had good self-perceived health and managed the transition to adulthood no differently to control subjects.
