Records from both planned and acutely admitted patients cared for in the Department of Surgery, Linköping University Hospital, were randomly chosen for scrutiny during 2009. Each month the records of 20 patients (about 9 % of all admitted) who were discharged, and all in-patients who deceased were examined to detect adverse events by using the Swedish adaptation of Global Trigger Tool (GTT). Any adverse event was noted in 20 % of all admitted, after surgery in about 30 %. During the first three months when patients with a hospital stay of at least 24 hours were randomly chosen adverse events were noted in 13 %. During the rest of the year when only patients with a hospital stay of at least five days were chosen randomly the corresponding figure was 23 %. Death was judged unavoidable in 28 patients, potentially avoidable in 5 patients and avoidable in 5 patients. Some of the findings have been starting points for various measures of improvement.
