Sammanfattning
A systematic review of the scientific support for dietary advice in diabetes has been performed. Primary outcomes were risk of cardiovascular and other diabetic complications and death (minimum follow-up 1 year) with weight and laboratory variables defined as secondary outcomes (minimum follow-up 24 weeks). In persons with impaired glucose intolerance, intense lifestyle intervention with a combination of low fat diet and physical activity reduces the risk of developing diabetes. In persons with diabetes, low fat (<30 E% from fat, 50-60 E% from carbohydrates) and moderate low carbohydrate (30-40 E% from carbohydrates) diets have similar beneficial effects on HbA1c and body weight. Studies of sufficient quality on long-term effects (including safety) of extreme low-carbohydrate diets (10-20 E% from carbohydrates) are lacking. Although there is some evidence of beneficial effects of vegetables, legumes and fish, there is no convincing evidence neither pro nor con a change of todays dietary recommendations in diabetes.
