Drug reviews in the clinical setting in Sweden usually describe their finding as Drug Related Problems. In risk and root cause analysis the terms Adverse Drug Event, Medication Error and Near Miss are used. This article describes different types of methods and terminology used by health care personnel working in the area of drug safety. Several methods are needed to promote a drug safety culture in the hospital and all the finding should be reported with the patient safety terminology to enhance the communication between the different processes and the collection of data.
