Thunderclap headache (TCH) is described as a sudden, unusually severe headache which reaches its maximum intensity within a split second or, at most, a minute. About 25% of TCHs can be associated with subarachnoid haemorrhages. A number of other disorders are known to present with TCH, e.g. cerebral venous thrombosis, cervical artery dissection, unruptured intracranial aneurysm, cerebral infarct, pheochromocytoma, reversible cerebral vasoconstriction syndrome and spontaneous intracranial hypotension. The established diagnostic assessment of TCH includes a computer tomography of the head and cerebrospinal fluid examination in order to exclude a subarachnoid haemorrhage. After exclusion of all possible causes, thunderclap headache may be considered a primary headache. The management of this type of headache is controversial. Articles differ in their conclusions and recommendations. The aim of this review is to discuss and highlight each of the presumed causes of thunderclap headache and discuss the clinical approach to thunderclap headache.
