Acute thrombosis of mechanical prosthetic heart valves is one of the major complications after valve replacement. Prophylactic antithrombotic treatment to avoid thrombosis traditionally consists of oral anticoagulants. Several recent reports highlight the potential effect of antiplatelet treatment in this setting. This report presents an overview of the field of mechanical heart valve thrombosis and describes a case of mechanical aortic valve thrombosis in a 70 year old man successfully treated with the combination of warfarin, aspirin and clopidogrel. Cinefluoroscopy verified improvement from mobility of only one leaflet to movement of both leaflets. Results from PROACT, an ongoing clinical trial will shed more light on the concept of antiplatelet therapy as prophylactic antithrombotic treatment in patients with mechanical prosthetic heart valves.
