Sammanfattning

We present the surgical outcome for men undergoing radical prostatectomy either with open surgery or robot-assisted laparoscopic technique during 2002 until 2006 at the Karolinska Universityhospital, Sweden. Patient questionnaires from men with a follow-up time of at least one year were analyzed. 411 open surgery and 768 robotic patients were evaluated. We went from operating all radical prostatectomies with an open technique to robot-assisted laparoscopy. Between 2002 and 2006 there was a decrease in positive surgical margins from 37 % to 22 %, erectile dysfunction from 66% to 60% while urinary incontinence rates did not change (10%). There was an increase in the part of men operated with “trifecta” from 19% to 28%. We could not find any indications that the introduction of robotic assisted laparoscopic prostatectomy deteriorates the situation for men diagnosed with prostate cancer. We cannot determine whether these positive surgical results would have been greater or less if instead a developmental program for open radical prostatectomy had been initiated.
