Waterpipe smoking is a tradition in the Middle East that has been considered quite harmless. The use of waterpipes is spreading among young Europeans, but with a different pattern of smoking habits. We describe two cases of carbon monoxide (CO) poisoning caused by waterpipe smoking. Both had a short period of unconsciousness after the smoking session and one had a seizure as well. One of the patients had a carboxyhemoglobin level of 32% on arrival to hospital. The other’s carboxyhemoglobin was not checked until 2h 40 min after the smoking and showed 21%. Both were treated with oxygen and recovered completely. Several explanatory factors contribute to the hazard of CO poisoning: a risk of higher CO content per smoke volume compared to cigarettes due to incomplete combustion, a larger volume of each waterpipe inhalation because of low and comfortable smoke temperature, and probably, a more intense way of smoking waterpipe among young Europeans.
