Kidney transplantation using organs from living donors (LD) has superior outcomes compared with organs from deceased donors. This publication describes the development of living donation and contains information about the rules regulating donor protection and social security. It also outlines how donors are selected today in comparison with earlier times. Finally, long-term results after kidney donation, e.g. blood pressure and data from a cross-sectional study of all kidney donors from Sahlgrenska University Hospital, Gothenburg, between 1965 and 2005 are presented. The most interesting finding is that the function in the remnant kidney increases for many years following donation. The paper concludes with hopes for the future as well as some comments from donors.
