Sammanfattning

Diabetes treatment is often sub-optimal during hospital admissions. Therefore, a proactive diabetes team (Diabetes Response Team) was set up at Hillerød Hospital in 2009. The purpose was to ensure uniform treatment for hospitalised diabetes patients regardless of department or specialty. Four experienced nurses are on call on weekdays for adult diabetes patients at every clinical department. Their supervision primarily encompasses guidance to care providers and patients with regard to insulin treatment. Team nurses independently propose minor insulin adjustments, while major changes are carried out under the supervision of endocrinologists. The number of cases supervised by the Diabetes Response Team is steadily rising. In the first six months of 2011 they supervised approximately 70 cases per month, of which approximately 80% were requested by neurology, oncology and surgery. The Diabetes Response Team plans to draft and implement clinical guidelines and participate in the work of describing patient trajectories in order to provide clear documentation and concrete diabetes treatment plans. Interdisciplinary specialist teams may be an option in other specialties with patients who have chronic diseases in addition to their specific diagnosis at admission.
