The development of radiotherapy for prostate cancer has been very rapid during the last few years. Modern conformal techniques have enabled reduced radiation to organs at risk and, as a consequence, less side-effects. Modern techniques have also made dose-escalation to the prostate possible. Local recurrences within treated volumes are therefore nowadays a rare phenomenon. Curative radiotherapy in combination with endocrine therapy results in superior overall survival as compared to endocrine therapy alone. Similarly, adjuvant radiotherapy results in increased overall survival after non-radical prostatectomy. Increased knowledge with respect to radiobiological characteristics of prostate cancer has led to new promising treatment concepts such as hypofractionation.
