Hiatus hernia affects hundreds of thousands of people in Sweden and is often associated with reflux disease. Mechanical problems such as esophageal dysphagia, bronchial complications and chest pain are at least as common symptoms of hernia as reflux. If gastroesophageal reflux is missing, a hiatus hernia becomes easily incorrectly classified as a radiographic incidental finding. After the successful introduction of proton pump inhibitors for reflux symptoms in the 1980s, remaining mechanical problems as indication for hiatus hernia surgery has tended to be underestimated. The recent development of Manoscan technique with high-resolution solid-state manometry systems has made it possible to display simultaneous recordings along the entire esophagus in colour-coded pressure plots which has improved the diagnostic assessment of hiatus hernia. We call for increased attention to the mechanical problems that can follow a hiatal hernia so that patients can get correct diagnosis for their symptoms.
