Patients =18 years with =5 prescribed medications in the medication list (ML) in the electronic medical record at 5 Swedish municipalities were recruited at scheduled check-up or invited by postal letter to participate and the actual prescribed treatment reported by patients´ was compared with the electronic medical record and the prescriptions stored in the Swedish national prescription repository. 223 patients gave written informed consent to participate, fulfilled the inclusion criteria and were interviewed. Of 2,263 prescriptions in the ML 76 % were current, 14 % non-current, 10 % duplicates and 8 % of the current prescriptions were missing. Of 1,908 prescriptions in the national prescription repository, 76 % were current, 13 % non-current, 11 % duplicates and 22 % of the current prescriptions were missing. The congruence between the three information sources was 52 %. More than 80 % of patients had at least one discrepancy in the ML, rendering a significant risk for mistakes and medication errors.
