Long courses of antibiotics have been a mainstay of acne treatment. The consequence has been the development of antibiotic-resistant Propionibacterium acnes (P. acnes). The antibiotic resistance seems to move from the acne patients to the community. The carriage of single and multiple antibiotic resistant P. acnes strains can occur in acne close contacts, but also in the general population. Oral antibiotics should be prescribed for a maximum of 3-4 months and should be discontinued when no more clinical benefit is noticed. Concomitant use of oral and topical antibiotics with chemically dissimilar properties which may select for multiple resistant P. acnes should be avoided. When repeated courses of an antibiotic are necessary,intervening courses of benzoyl peroxide are recommended in order to reduce/eliminate resistant propionibacteria. Once the inflammatory lesions resolve the antimicrobial treatment may be discontinued and maintained with topical retinoid monotherapy or a combination with a topical retinoid and benzoyl peroxide.
