Deep venous thrombosis (DVT) is a frequently suspected condition where non-specific symptoms are common and clinical diagnosis is often difficult. While awaiting the results of ultrasonography or venography, anticoagulant treatment is initiated in many patients with a high risk of DVT. We report on two cases with compartment syndrome and one case with significant calf muscle haematoma in patients receiving low-molecular-weight heparin (LMWH) for suspected DVT. Surgical intervention was required in all cases. Ultrasonography with compression test of proximal veins is a rapid and non-invasive method to rule out a proximal DVT. If performed in the emergency room, an improved clinical assessment can be achieved during on-call hours. Pre-diagnostic initiation of anticoagulant treatment and the associated risk of bleeding can thereby be reduced.
