Background: Norwegian hospitals have recently implemented Joint Care: accelerated patient care related to hip and knee arthroplacy. Joint Care includes preoperative education, teamwork, patient participation and effective pain management. The objective is to re-establish normal functions after surgery. Previous studies show a positive medical and cost-effective benefit. However, more knowledge is needed regarding the interaction between patients and nurses in the early postoperative period.
Objective: The purpose of this study is to describe nurses’ experiences with Joint Care, and how nurses cooperate with patients on postoperative pain management.
Design and methods: The study has a qualitative descriptive design with five semi-structured interviews of nurses. Data-analysis used principles from descriptive qualitative methods.
Results: Patients are mobilized early after surgery, but several experienced severe pain at mobilization on the second postoperative day. Patients usually use a passive form of communication, and this study indicates that nurses and patients have different assumptions about using the "numeric rate scale" as a pain-assessment tool.
Conclusion: The implementation of accelerated patient care requires not only a reorganization of procedures and routines. Action should ensure that incorporated perceptions and role pattern of health personnel and patients would change and hopefully improve patients’ postoperative pain management.
