Retropharyngeal calcific tendinitis (RCT) is a rare, but most likely, an under-recognized condition where calcium hydroxyapatite deposition in the longus colli tendon induces an acute inflammation of the longus colli muscle and tendon with neck pain, dysphagia, odynophagia, limited cervical motion and low-grade fever. Laboratory tests may demonstrate mild leukocytosis and elevated erythrocyte sedimentation rate. The disease may mimic more serious causes of acute neck pain such as retropharyngeal abscess, meningitis and traumatic injury. A definitive diagnosis can be established using imaging studies, which show the pathognomonic calcifications anterior to C1 and C2 and prevertebral soft-tissue swelling between C1 and C4. Management is based on administration of non-steroidal anti-inflammatory medication.
