Hashimoto encephalopathy is a complex disease with a variety of neurological and psychiatric symptoms such as seizures, hallucinations, confusion and pareses. When other possible causes are excluded, Hashimoto encephalopathy is a clinical diagnose in combination with elevated anti-thyroid antibodies. The diagnosis is strengthened by EEG-findings, clinical response to corticosteroids and elevated protein levels in CSF. Hashimoto encephalopathy was originally considered a complication to Hashimoto thyroiditis. Later the disorder was found in individuals without a thyroid disease but still with elevated anti-thyroid antibodies. The pathogenetic mechanism and the actual role of the antibodies in the disease are unclear. This case report describes an 11-year old girl with a sudden onset of seizures and neuropsychiatric symptoms. After extensive investigations she was diagnosed with Hashimoto encephalopathy. She responded well to steroid treatment and has recovered completely. Hashimoto encephalopathy should be considered as a differential diagnosis in unexplained seizures, encephalopathies and rapid mental decline.
