Bronchopleural fistula (BPF) is a rare, but feared complication to pulmonary diseases such as pneumothorax, lung abscess or after thoracic surgery for malignancy. BPF is associated with high morbidity and mortality. Adequate pleural drainage together with aggressive antibiotic treatment and surgical intervention is the conventional approach. When this fails, various bronchoscopic methods have been tried. Recently, endobronchial vent placement was introduced as a promising bronchoscopic intervention for emphysema. We report four cases with successful closure of chronic BPF using endobronchial vents. Underlying diseases were emphysema with pneumothorax, spontaneous pneumothorax, and lung abscess after surgery for lungcancer. All patients had required persistent long-term pleural drainage. This could be removed a few days after vent placement, and no recurrences of BPF were noted. Use of endobronchial vents is an effective, minimally invasive intervention for patients with prolonged pulmonary air leaks.
