Synchronous multiple primary melanoma is a poorly recognized problem that occurs in everyday clinical practice. In patients with primary thin melanoma the occurrence of a new melanoma is more common than a recurrence of the primary melanoma. The second melanoma is often discovered at an earlier stage (thinner) than the first. The value of early detection is high by reducing the risk of tumor metastasis and thereby significantly improving patient survival. The present article summarizes the state-of-the-art and presents data on multiple primary melanomas diagnosed between 2006 and 2007 at the Karolinska University Hospital. Seventeen multiple primary melanomas were found among 163 operated melanomas, six of which we discovered during surgery and another 11 during follow-up the following year. We recommend that in all patients who had surgery for a malignant melanoma, complete examination of the skin is performed by a dermatologist with special interest in the diagnosis and management of skin tumours and pigmented lesions in connection with the treatment of primary melanomas.
