The well-known fear-avoidance model is often used to describe how patients with chronic pain creates a negative behavioral spiral by avoiding physical and social activities, and thereby continuously worsen their physical disabilities and other symptoms. The model can be extended to also include avoidance of rest and recuperation as a mechanism for pain development. This article focuses on the group of patients who engages in activities but avoids necessary rest and recuperation. An example case is described and we detail a rehabilitation program specifically designed to reach these types of patients. Finally a number of patterns that may increase the likelihood of developing an endurance behavior are listed. The patterns include physical activity used as a means of creating personal identity, use of exercise to relieve anxiety, inability to accept physical limitations and the experience of increased pain when resting.
