QTc-prolongation in acute drug-poisoning. Guidelines for management from the Swedish Poisons Information Centre
Torsade de pointes is a rare complication of drug-poisoning in Sweden. QTc-prolongation on the ECG is, however, a common finding in cases of poisoning and the Swedish Poisons Information Centre often receives inquiries concerning the optimal management of such patients. In this article, a recent case of sotalol poisoning complicated by torsade de pointes is reported and new guidelines for management of QTc-prolongation in acute drug-poisoning are presented. The recommendations are based on a systematic review of the literature and consist of an assessment of important risk factors such as type of ingested drug, extent of QTc-prolongation, heart rate and co-existing electrolyte disturbances. Clinical management decisions such as indications for continuous arrhythmia monitoring and prophylactic pharmaceutical therapy to prevent torsade de pointes are discussed.
