Drug utilization review – a practical example from primary care
This primary care case study describes a drug utilization review (DUR) on a 77 year-old multi-morbid patient with stupor, tremor and absences. The patient was treated with 15 different drugs on a regular basis and his symptoms were consequently interpreted as adverse drug reactions (ADR). Nine drugs were stopped due to the following reasons: lack of indication (5), ADR (2), ADR and lack of indication (1), drug-drug-interaction and lack of indication (1), doubtful risk-benefit-ratio (1). Follow-up was time consuming as every change in drug treatment had to be re-evaluated. The number of emergency visits and hospitalisations decreased from six in the year before DUR to two in the year after. Questioning indications for drug treatment, actively searching for ADR and following up on changes in drug treatment were important steps in optimising this patient’s drug utilization.
