Pheochromocytoma is a catecholamine producing tumor originating from the adrenal medulla. The characteristic symptoms are recurrent attacks of headaches, palpitations and perspiration. Here we discuss a 39-year-old woman with pheochromocytoma who presented with severe acute congestive heart failure and cardiogenic shock. The diagnosis of pheochromocytoma was confirmed by analyses of catecholamines and metanephrines in urine and plasma after an adrenal mass was incidentally detected on a thoracic computed tomography. After preoperative treatment with alpha-receptor antagonist, the patient was successfully treated by laparoscopic adrenalectomy with subsequent complete restoration of her cardiac function. The case demonstrates that not all patients with pheochromocytoma present with the classical episodic symptoms of catecholamine excess. In patients with heart failure of unknown etiology, pheochromocytoma should be considered in the diagnostic work-up.
