Sammanfattning

In Afghanistan Swedish soldiers are exposed to situations with a high risk of being injured in combat. Battlefield injuries in Afghanistan are commonly caused by improvised explosive devices (IED) and gunshot wounds (GSW). The majority of battlefield casualties die in the pre-hospital environment and haemorrhage remains the leading cause of potentially survivable death. The Swedish military has implemented the Battlefield Advanced Trauma Life Support (BATLS) concept. This includes, among other things, early control of catastrophic haemorrhage, measures against airway obstruction and decompression of tension pneumothorax. Severely injured patients are evacuated from the battlefield with the forward medevac UH-60 Black Hawk helicopter, staffed with an anaesthesiologist and an EMS nurse. For military operations far away from the field hospital Sweden has a Forward Resuscitation Team (FRT). The FRT is a small mobile physician-staffed unit for advanced resuscitation. If needed the FRT can be supplemented with a surgeon, surgical nurse and operating room tent to form a Forward Surgical Team (FST) with the ability to perform damage control surgery. This article aims to discuss the battlefield injury panorama, BATLS management, equipment and the structure of the Swedish battlefield medicine including helicopter medevac in Afghanistan.
