The article describes a quality development project that tested a new nurse-led intervention that aimed to ensure proper medication of elderly patients with multiple chronic illnesses. The intervention consisted of a specialised review of medication on admission and adaptation of medication upon discharge by the case manager, who also followed the patient at home, examined their medication at home and ensured the patient's understanding and handling of the medicine. A number of weaknesses and errors were identified and corrected, and it became apparent that there was a clear need for the nursing coordinator, as numerous problems calling for intervention were identified on arrival at home.
