Total or subtotal avulsion of the hamstring insertion at the ischial tuberosity is fairly uncommon but may require early surgery in order to
achieve the best results and least morbidity. The injury is seen among younger elite athletes, but the majority of patients are middle aged
recreational athletes or non athletes. Early MRI and clinical examination helps in the assessment of the need for surgery. When at least 2 out of
the 3 tendons are avulsed with minimum 2 cm retraction surgery should be considered. The results of early surgery are good and the morbidity
low. A delay in surgical repair renders the repair more technically challenging, may increase the likelihood of sciatic nerve involvement, and
reduces postoperative outcome in terms of hamstring strength and endurance. Brace is seldom used except in some chronic cases. Once the
nature of the injury has been established, the surgical treatment of hamstring origin avulsions has predictable and satisfactory results despite a
fairly long rehabilitation period of physical therapy.
