Main message of the article: Systematic identification of symptoms in palliative in-patients and
action-supporting work instruments develop the skills and knowhow of the nursing staff.
Documentation using this same systemization facilitates overview and follow-up of planned and
implemented care measures.
Methods: Development and testing of instruments for symptom screening, instructions for action and
documentation in palliative patients admitted to hospital. Journal audit on eight patients.
Questionnaire among care staff (n=45) concerning own skills within palliation and regarding the use
of such tools.
Results:The tools support, inspire and aid continuity in the care.
Screening can be overwhelming, but appears to focus more attention on invisible symptoms.
More training in palliation is desired.
Nursing implications: Increased focus on the development of nursing for palliative patients and the
prioritisation of time for such work. Development of skills and knowhow within palliation among the
nursing staff.
