Many elderly patients have decreased renal function. Plasma creatinine levels may be within the reference range despite decreased renal function due to reduced
muscle mass. It is important to estimate (e) the renal function as e-creatinine clearance or eGFR for adjustment of pharmacotherapy. We have developed a computerized
decision support system (CDSS) called the »Renal button«. This function estimates a patient’s renal function provided at the moment of prescription and presents a »web
window« with suggestions for appropriate drugs and dosages for three different levels of renal function. The first advice is short, with longer explanations accessible in link
windows. Our model of providing reminders about dosages was appreciated by the prescribers. Its simple pedagogic design can illustrate how other similar CDSS’s can be
created to provide the overview needed and time for providers to reflect about decision and dosage of drugs. The CDSS is of help when therapeutic drug monitoring (TDM)
cannot be used.
