Localized buccal gingival recessions (GR) can cause problems related to esthetics, root dentin
hypersensitivity and gingival inflammation. Different techniques have been shown to be effective in
the treatment of such defects on a short-term basis, but little is published regarding long-term effects.
The aim of this review was to identify which technique yields the best result for the treatment of GR
(Miller class I and II) in a long-term perspective ( 5 years). A search in PubMed identified three
publications including three treatments fulfilling the inclusion criteria. These treatments documented
the effect of coronally advanced flap (CAF), CAF + autogenous connective tissue graft (CTG), and free
gingival graft between 5 - 14 years. The data available was very limited, and direct comparison of the
treatments was difficult. CAF and CAF+CTG showed the best results regarding root coverage and
reduction in recession height.
Conclusion: Within the limitations of the present study, CAF and CAF+CTG showed the best results for
the treatment of GR in a long-term perspective, but the results seem to decrease over time. However,
the decrease seems to be reduced by careful preoperative patient- and site selection, systematic
postoperative care, and optimal surgical execution.
