It can be problematic to distinguish between the altered states of consciousness that may occur
following acquired brain injury. To inform non-specialist clinicians and next-of-kin, this article
introduces the relevant methods for assessing consciousness. The current clinical standard is to
monitor for behavioral signs of consciousness using the checklist scale CRS-R. In addition,
neurophysiological methods have shown promise, but most of this work has not yet been validated
for clinical use. The current operational definition of awareness may be biased toward classifying
behaviour rather than the phenomenological experience. Studies have suggested that behavioural
assessment underestimates the level of consciousness, and patients failing to show behavioural
signs of awareness could benefit from a more thorough multicentre and multi-method examination.
